OCT – Optical Coherence Tomography is a light-based catheter which acquires on an image (photo) inside the heart blood vessel. OCT is a recently-developed, catheter-based Intravascular Imaging Technology that provides micron-scale resolution. It is a laser light equivalent of ultrasound imaging, measuring the intensity of backscattered infrared light rather than sound waves, and translates these optical echoes into a high-resolution, two-dimensional tomographic image. OCT has the highest imaging resolution of any currently-available technology. Cross-sectional resolutions of catheter-based OCT systems are in the range of 10-20 µm.

Carpal Tunnel Syndrome (CTS) is a condition characterized by numbness, pain, and discomfort in the hand, often accompanied by tingling, burning sensations, weakness in hand grip, or persistent pain. It occurs due to the gradual compression of the median nerve by surrounding structures within the carpal tunnel of the wrist, leading to nerve entrapment symptoms. These symptoms typically affect the thumb, index, and middle fingers, although the pain can sometimes radiate up to the shoulders.
Approximately one in five individuals experiencing these symptoms may be diagnosed with CTS following a thorough clinical examination, imaging, and electrophysiologic (nerve conduction) studies. If left untreated, CTS can lead to permanent nerve damage, resulting in constant numbness and muscle wasting in the hand, particularly affecting thumb movements.
CTS can affect both hands, although symptoms may be more pronounced on one side. In the early stages, symptoms are often intermittent and tend to occur at night, likely due to the flexed position of the wrist during sleep. The numbness can be intense enough to disrupt sleep.
Non-surgical treatment options include the use of night splints, steroid injections, and oral anti-inflammatory and analgesic medications. However, in acute cases, surgery may be the only viable option. The procedure, performed under local anesthesia, typically lasts no more than 30 minutes, with mild sedation helping to expedite the process.
Using an endoscopic technique with the assistance of a video monitor and a Smart Release device, the surgeon precisely cuts the transverse carpal ligament from underneath, avoiding a large incision on the palm. The retractable blade is then withdrawn, and the less-than-1 cm incision is closed with absorbable sutures.
The result is a small, cosmetically acceptable scar hidden within the wrist crease. Patients can typically return to work within eight days or even sooner.
